Livability Home Health:
My Journey of Compassionate Care
I’m San Pham, the founder of Livability Home Health, and my journey has been driven by a deep commitment to enhancing the well-being of our community.
Livability Home Health was born out of a mission to provide affordable, high-quality care to the disabled and elderly populations in New Hampshire.
My passion for home care services was shaped early on in life. Growing up, I spent a great deal of time with both of my grandmothers, acting as their companion. This experience gave me a unique appreciation for the wisdom and warmth of the elderly, and I discovered that I formed natural bonds with people of all ages. These meaningful connections, combined with my passion for caregiving, led me to pursue a career in healthcare.
After earning my Bachelor of Science in Healthcare Management from Northeastern University in 2013, my final project evolved into Livability Home Health. With over 15 years of experience in the field, my dedication to compassionate care has only grown stronger. My service will continue to support New Hampshire's senior citizens while also planting the seeds for providing my own parents with the best care possible when the time comes—just like we do for our clients.
Through Livability, I am honored to follow my passion by offering care that reflects both the needs and dignity of those we serve. Join me on this beautiful journey of compassionate care and community well-being.

I’m proud to share that I am working toward my graduate's degree at University of New Hampshire 2024, which has equipped us to expand our expertise and services even further!
We are thrilled to announce that Livability is entering an exciting phase of growth, with new clinical services on the horizon, including expanding our services to Florida.
